De l\u2019introduction des GMFs dans le d\u00e9but des ann\u00e9es 2000, \u00e0 la cr\u00e9ation des CISSS et CIUSSS, \u00e0 l\u2019abolition des Agences R\u00e9gionales et derni\u00e8rement la cr\u00e9ation de Sant\u00e9 Qu\u00e9bec sous la loi 15, toutes des r\u00e9formes qui ont eu un impact mitig\u00e9 sur le syst\u00e8me de sant\u00e9. \u00c0 travers tous ces changements, nous avions une certaine confiance que gouvernement savait o\u00f9 il s\u2019en allait et avait notre bien-\u00eatre \u00e0 c\u0153ur. Or, avec l\u2019adoption de la loi 2 (sous b\u00e2illon doit-on le rappeler), cette confiance est perdue et nous ne pouvons plus nous taire.<\/p>\n\n\n\n<p>La loi 2 est aussi complexe (le Ministre Dub\u00e9 tarde \u00e0 clarifier plusieurs aspects lorsque les m\u00e9dias et les m\u00e9decins lui demandent des pr\u00e9cisions) qu\u2019antid\u00e9mocratique (une loi adopt\u00e9e sous b\u00e2illon \u00e0 04h00 du matin dans la nuit de vendredi \u00e0 samedi!? Quelle est cette pratique digne de l\u2019\u00e8re Sovi\u00e9tique !?). En utilisant des termes tendances, comme \u00ab capitation \u00bb, \u00ab performance \u00bb, \u00ab p\u00e9nalit\u00e9s \u00bb, le gouvernement Caquiste tente de s\u2019arracher une portion de l\u2019opinion publique en essayant de se montrer s\u00e9v\u00e8re dans ses n\u00e9gociations avec les m\u00e9decins (\u00e0 la veille d\u2019une \u00e9lection non moins!).<\/p>\n\n\n\n<p>\u00c9tant une agence de facturation m\u00e9dicale (une compagnie qui g\u00e8re la paie des m\u00e9decins dans le syst\u00e8me public), nous regardons quotidiennement \u00ab sous le capot \u00bb du syst\u00e8me de sant\u00e9 qu\u00e9b\u00e9cois, ce qui nous permet d\u2019avoir des connaissances approfondies des termes tendances qu\u2019utilise le gouvernement. Ces termes ne sont pas nouveaux, malgr\u00e9 que le Ministre Dub\u00e9 se pette les bretelles avec sa r\u00e9forme drastique, et nous tenons \u00e0 vous vulgariser leurs d\u00e9finitions :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>\u201cCapitation\u201d<\/strong> \u2013&gt; Cela veut dire qu\u2019une portion de la r\u00e9mun\u00e9ration des m\u00e9decins de famille sera li\u00e9e au nombre de patients qu\u2019ils auront inscrits. Plus le nombre de patients inscrits est \u00e9lev\u00e9 (modul\u00e9 aussi selon la complexit\u00e9 des patients inscrits), plus la r\u00e9mun\u00e9ration sera importante. <strong>Ceci n\u2019a rien de nouveau ! Les m\u00e9decins de famille ont d\u00e9j\u00e0 une portion non-n\u00e9gligeable de leurs revenus qui sont li\u00e9es \u00e0 ce crit\u00e8re pr\u00e9cis!<\/strong><\/li>\n\n\n\n<li><strong>\u201cPerformance\u201d<\/strong> Les docteurs, qu\u2019ils soient pay\u00e9s \u00e0 l\u2019acte, \u00e0 l\u2019heure, ou dans un mode mixte, sont d\u00e9j\u00e0 pay\u00e9s pour leur performance. Plus qu\u2019ils font d\u2019heures, ou d\u2019actes, plus ils sont pay\u00e9s! <strong><u>Encore une fois, rien de nouveau !<\/u><\/strong><\/li>\n\n\n\n<li><strong>\u201cP\u00e9nalit\u00e9s\u201d<\/strong>\u2013&gt; Il existe d\u00e9j\u00e0 plusieurs restrictions administratives qui encadrent le travail des m\u00e9decins. Ces derniers doivent avoir un PREM (un permis pour travailler dans une r\u00e9gion g\u00e9ographique pr\u00e9cise), doivent se conformer aux AMPs (activit\u00e9s m\u00e9dicales particuli\u00e8res qui sont moins attrayantes et octroy\u00e9es selon la n\u00e9cessit\u00e9 locale par leur DTMG), doivent subir des plafonds de revenus et d\u2019activit\u00e9s (des limites sur le montant d\u2019argent et \/ ou le nombre de certains actes m\u00e9dicaux selon une p\u00e9riode donn\u00e9e au-del\u00e0 desquels ils ne sont pas pay\u00e9s), etc. Si les m\u00e9decins ne respectent pas ces contraintes, ils sont assujettis \u00e0 des p\u00e9nalit\u00e9s s\u00e9v\u00e8res pouvant allant jusqu\u2019\u00e0 30% de leurs revenus bruts. Et donc, imposer des p\u00e9nalit\u00e9s administratives n\u2019a rien de nouveau! <\/li>\n<\/ul>\n\n\n\n<p>Maintenant qu\u2019on s\u2019entend que ces termes tendances n\u2019ont rien de nouveau, il semble que les seules choses \u201cdrastiques\u201d de cette r\u00e9forme sont la fa\u00e7on dont la loi a \u00e9t\u00e9 adopt\u00e9e (sous b\u00e2illon, \u00e0 04h00 du matin la nuit de vendredi \u00e0 samedi) et le niveau de dommage potentiel que subira notre syst\u00e8me de sant\u00e9 si la loi reste inchang\u00e9e (avec la perte de centaines de m\u00e9decins vers l\u2019Ontario et ailleurs). Pourquoi l\u2019adopter sous b\u00e2illon si elle est vraiment bien fond\u00e9e et logique? Pourquoi ne pas laisser le temps aux processus habituels, qu\u2019est-ce qui presse (\u00e0 part la date de la prochaine \u00e9lection)? <\/p>\n\n\n\n<p>Nous avons vu plusieurs exemples de comment le Ministre Dub\u00e9 va couper certains co\u00fbts avec la loi 2 \u2013 ce qui est \u00e9videmment une bonne chose \u2013 mais, comment ces coupures vont-elles am\u00e9liorer le syst\u00e8me de sant\u00e9 vraiment? Comment, concr\u00e8tement, la loi 2 est-elle b\u00e9n\u00e9fique pour les Qu\u00e9b\u00e9cois? Quel article de cette loi permettra un temps d\u2019attente moins long aux Urgences? Ou de r\u00e9duire la liste d\u2019attente pour une chirurgie? Ou d\u2019obtenir un m\u00e9decin de famille pour chaque qu\u00e9b\u00e9cois?<\/p>\n\n\n\n<p>Nous sommes tous d\u2019accord que le syst\u00e8me de sant\u00e9 a besoin d\u2019aide \u2013 <strong><u>mais nous avons besoin de solutions concr\u00e8tes, r\u00e9alistes, et pr\u00e9cises qui sont bas\u00e9es sur une collaboration entre les experts des deux c\u00f4t\u00e9s de la table de n\u00e9gociations, et non pas une d\u00e9cision ex\u00e9cutive,<\/u><\/strong> unilat\u00e9rale et forc\u00e9ment politique, qui r\u00e9sulte de la vision \u00e9troite d\u2019un parti politique qui vise avant tout la r\u00e9\u00e9lection.<\/p>\n\n\n\n<p>Si le gouvernement Legault ne veut pas \u00eatre connu comme celui qui a d\u00e9truit notre syst\u00e8me de sant\u00e9, en autre, en causant un exode massif de m\u00e9decins vers l\u2019\u00e9tranger, <strong><u>il doit suspendre l\u2019application de la loi 2 et retourner n\u00e9gocier de bonne foi avec la FMOQ\/FMSQ<\/u><\/strong><\/p>\n\n\n\n<p>Sinon, et je ne veux pas avoir un ton trop alarmiste, mais, avec les m\u00e9decins actuels qui quittent le Qu\u00e9bec, et avec l\u2019attrait de ce m\u00e9tier qui vient de chuter pour les prochaines g\u00e9n\u00e9rations, j\u2019ai bien peur que \u00e7a pourrait devenir le d\u00e9but de la fin de notre syst\u00e8me de sant\u00e9 public.<\/p>\n\n\n\n<p>Patrick Busch, Pr\u00e9sident MEDIREGI BILLING SERVICES INC.<\/p>\n\n\n\n<p><a href=\"mailto:patrick@mediregi.ca\">patrick@mediregi.ca<\/a><\/p>","protected":false},"excerpt":{"rendered":"<p>MEDIREGI is a RAMQ medical billing agency founded in 1987 (www.mediregi.com), so we\u2019ve seen our share of healthcare reforms.
